Your first visit establishes a vital foundation for our relationship. ​

Please bring the following to your first office visit:

  • Identification Card (Driver's License or State ID)

  • Insurance Card(s)

  • List of current medications

  • Any past relevant medical records, including foot x-rays

  • Referral from your primary care physician (if required by your insurance)

  • Completed New Patient FORMS
